Steps to get a Social Security Number (SSN):
- Go to the Social Security Administration office at 175 East 400 South, Suite 500 (phone #: 801-524-4115) in Salt Lake City
- Fill out Form SS-5 and take it to a clerk
- Show your passport to the clerk
- Show your I-94 and IAP-66 to the clerk
- Request a receipt
- Ask if you can call to get the SSN. If not it will be mailed to you in about two weeks
If you are told you do not qualify for an SSN, tell them you are an F-1 student (or J-1 student, exchange scholar, or whatever) and need the SSN in order to open a bank account in the U.S. and file a tax return as required by the Internal Revenue Service. If you are an F-1 or J-1 student and have on-campus work authorization from the International Center, show that to them.
When you receive your SSN, please email it to Sara Emery. You also need to give the SSN to:
- Your Department
- Payroll Office
- Registrars Office
- Human Resources
