Visitor Vendor Number Advance Request

Instructions: Please submit the following information to request a vendor number in advance of a visitor’s arrival. This form may speed up any reimbursement process.

NOTE: We must have either a home or actual work address for the visitor, this information will help to identify the individual without using private identification numbers.

Your department may still choose to have the reimbursement check sent to the University of Utah campus address.
