If any pre-payments have been processed, including airfare, please fill out a final reimbursement. You will need to contact our onsite travel agency to cancel the airfare.

Please fill out this form if any of the following scenarios apply:

1:

  • Travel dates are past, and
  • Travel did not occur, and
  • There are no pre-paid expenses other than the audit fee, and
  • Trip should be closed.

2:

  • Or, the travel is a future trip that will not take place.
