W-2, IRS Form:
Discussion: The University of Utah must issue to the employee and report to the IRS Form W-2, wage and tax statement. Wages, federal, state income taxes, Social Security and Medicare taxes, as well as certain additional information is reported on Form W-2.

W-4, IRS Form:
Discussion: Form W-4, Employee’s Allowance Withholding Certificate, is filled out by employees so that the University can withhold the correct amount of federal and state tax from wages. Employees can change their Form W-4 through the campus information system.

W-8BEN, IRS Form:
Discussion: This form is the equivalent of a W-9 but is for foreign individuals who are not Legal Permanent Residents (formerly a Green Card Holder) or qualify as a “Resident for Tax Purposes”. This form certifies their foreign status and is used most often for foreign short-term visitors.

Discussion: The University uses this form to request the taxpayer identification number (TIN) of a US person (including a resident alien) and to request certain certifications and claims for exemption.

Web Journal Processing:
Discussion: Web Journal Processing is the means whereby accounting data (generally billing transactions for services provided) is electronically passed to the University’s general ledger from a method utilized by a department to facilitate its business processes.

Write-Off Authorization Letter:
Discussion: A letter sent to a department’s Inventory Controller that lists all assets identified as LOST during the annual capital inventory. The letter requests a memo to be written explaining efforts made to locate the missing equipment and signatures of the Department Head, Dean/Director, and Cognizant Vice-President authorizing the write-off and removal of the listed assets from the department inventory.
