Financial and Business Services is excited to announce the addition of the “Payment Card Acceptance/eCommerce” pages to its Web Presence.  You can view the new content at http://fbs.admin.utah.edu/index.php/payment_card.  This additional information will be helpful for departments across the University,  as they engage in business transactions using either Payment Cards or eCommerce.

Two new FORMS pages have been added to the FBS web presence.  This will enable visitors to have access to all the FBS forms in one place.  The forms are sorted alphabetically and by department.  A FORMS link on the top row of the FBS website has been added.  This will make it easy to find the form you need especially if you are not sure which department to look in for that form.  This is similar to the FORMS pages in the FBS pagelet in CIS.  On September 15, 2008, the FORMS links on the FBS pagelet will be changed to direct users to the new FBS FORMS pages.

The Utah State Tax Commission has assigned a new sales tax license and exemption number to the University of Utah.  Please begin to use sales account license and sales tax exemption number 11874443, replacing 20203, immediately.  A new sales tax exemption certificate, Form TC 721, has been posted to the Purchasing website at http://www.purchasing.utah.edu/forms/tc-721.pdf.  Please provide this new updated form to our suppliers.

Effective April 1, 2008, several Utah counties and cities have an increased sales tax rate.  If your department conducts business in Richmond, River Heights, Cache County, Bountiful, Centerville, Blanding, Naples or Weber County, please see the increased sales tax rate at http://tax.utah.gov/sales/ratechanges.html.

The Utah Legislature amended the way that Utah income taxes are calculated. Accordingly, there is a new 2008 payroll tax withholding table. The new table will be in effect beginning with the pay period ending 3/15/08.

You may notice a minor difference in your Utah income tax withholding on the paycheck issued on Mar 21st, 2008 compared to prior paychecks, even though your tax withholding status has not changed.

Form W2, Wage and Tax Statement for year 2007 will be delivered by January, 31st, 2008 to current employees’ home departments and mailed via the U.S. Postal Service to former employees.Reprinted Forms W2 for year 2007 are available on line.  Employees or former employees can reprint their own W2 by going to the University Campus Information System at: CIS.UTAH.EDU The navigation for this feature is as follows: 

  • In the Campus Information System, go to the Employee Tab.
  • Click on the link My Human Resources/Payroll
  • Click on W-2 Form Reprint.

The employee will then be able to reprint the W2 by entering year 2007.  If employees or former employees do not remember their password, they may contact  the Campus Help Desk at (801) 581-4000.  If the employees or former employees do not remember their University id, they should contact their (former) home department.  If an individual would like Tax Services to re-print his or her W2 copy, a W2 reprint request form is required with a $12.00 reprint fee.  The reprint form is available at WWW.TAX.UTAH.EDU.  The 2007 W2 reprint request will be processed in two business days. Some YTD totals on the online “View Paycheck” for the 12/21/2007 payday were temporarily incorrect. The error was discovered and has been corrected.

This did not affect the pay period gross or net amounts.

Please advise your employees in case they have already viewed (and possibly printed) their online check/advice.

Effective 1-1-2008, the state portion of the sales tax rate will be reduced from 4.75% to 4.65%, and for Salt Lake County the mass transit portion of the sales tax rate will be increased from .25% to .30%. The net effect to the Salt Lake City sales tax rate is a reduction from 6.85% to 6.8%. For changes to the sales tax rates for other locations, see the Utah State Tax Commission sales tax table at:

Also effective 1-1-2008, the tax on food and food ingredients has been reduced to 3.0% statewide. For a reminder as to which food items qualify for this special rate, see:

Tax Services would like to announce the following personnel changes:

Randy Durney has been promoted to Senior Accountant. Randy’s responsibilities will include preparing payroll tax deposits, preparation of employment tax returns, handling student FICA exemption issues, payroll tax questions, and coordination of Form W2 processing.

Wendy Kwan, formerly a part-time employee, has accepted the position of Accountant, beginning July 23rd. Wendy will meet payroll and other deadlines with respect to garnishments, tax levys, child support, voluntary wage assignments, educational assistance garnishments and will review monthly sales tax returns.

Please join us in congratulating Randy and Wendy on their new appointments!

Accounts Payable would like to announce the following personnel changes:

Judy Chan has returned to Accounts Payable as a supervisor over our support services areas. Her responsibilities include check distribution, document imaging, Escheat and 1099 reporting audits, payment management, quality control, the reception area and vendor master maintenance.

Vickie Allen is an Associate Accountant processing vendor invoices for payment. Vicki has worked in AP since August 2005 as a scanner for the Fortis imaging system. Vicki is a Junior at the University studying Health Promotion and Education.

Effective April 1, 2007, there are new sales tax rates effective for Salt Lake County. The new base sales tax rate is 6.85% for Salt Lake County. If you do business in another county, please check the Utah State Tax Commission table for April 1, 2007 sales tax rates:

Reprinted Form W-2s for year 2006 are now available online!

You can print your own year 2006 W-2 by going to the CIS at http://cis.utah.edu/.
